The government’s response called for increased demands on healthcare providers (Under a review of the Care Quality Commission) to deliver transparent auditable processes for monitoring incidents and complaints and provide frameworks for patients and healthcare professionals to feel comfortable in highlighting issues / failures.
Delivering healthcare in patients’ own homes as opposed to a hospital environment presents unique challenges in managing quality & risk. Remotely monitoring the quality of care delivered, and trapping complaints or incidents, requires special mobile solutions to enable capture of information as quickly as possible and as near to the point of delivery of care.
Project E-vita Patient Record system from Evaware Ltd has two features fully integrated into the patient record to assist in tackling these problems. Firstly what we termed “Careplan” functionality and secondly Incident & Complaint Management.
Careplans in Project E-vita are a mechanism for the establishment of a planned pathway of clinical and administrative procedures for a patient through to their eventual discharge. Deviations or exceptions are centrally monitored on a “service dashboard”. The entire pathway is mapped out in the Careplan and ad-hoc tasks can be added as the patient progresses through the treatment process. The service dashboard integration gives management a powerful oversight on what services have or have not been delivered to patients in a live feed environment and enabling them to make decisions on how best to deal with any exceptions.
Incident & Complaint Management
Careplans are designed to minimise the chance of error and are the first line of defence in managing risk in healthcare environments through there is always the chance of something going wrong. Incident and Complaint management make it easy for healthcare professionals to safely record directly onto a patient record that an incident has occurred or that a complaint has been received from a patient or their relatives. As with Careplans, Incident and Complaint cases are fully integrated into the service dashboard to quickly raise a flag with the team management that an issue has arisen and start a chain of events to manage the situation. Cases are followed up in a timely manner using the system’s review cycle and activities (Telephone calls, letters written etc) are all recorded in a comprehensive audit log providing a transparent and confidential trail showing the steps taken to resolve the situation.
